Opening an aesthetic clinic? Here is how the space is zoned, what laser rooms need, which finishes work and what the fit-out costs.

An aesthetic clinic or medispa is two businesses under one roof: a medical clinic, which must be designed to Ministry of Health (KKM) standards before it can be registered, and a spa-style experience that sells the treatment. The fit-out has to keep clinical rooms, clean and dirty areas and medicine storage in order while every surface still looks premium. ClickBina's clinic guide puts aesthetic and specialist clinics at RM250 – RM350 per sq ft, from around RM200,000, with approvals adding RM3,000 – RM5,000 (indicative 2026, Klang Valley).
A general practice clinic is built around consultation and basic treatment. An aesthetic clinic adds procedures such as injectables, lasers and energy-based devices, skin treatments and sometimes minor surgical work, all sold to clients who expect a hotel-like experience. That combination drives the design: more treatment rooms than consultation rooms, heavier power and ventilation in some of them, secure storage for medicines and consumables, and a front of house that feels like a lounge rather than a waiting room.
Where treatments are medical, the premises are a clinic in law. The clinic interior design guide covers the rules in more detail. In short, clinics are regulated by KKM under the Private Healthcare Facilities and Services Act, and the design must satisfy KKM standards before the clinic can be registered and operate. A beauty spa offering only non-medical treatments is a different kind of premises, so decide early which one you are building.
| Zone | What it is for | Design points |
|---|---|---|
| Reception and lounge | Arrival, registration, payment, retail shelf | Private consultation corner so clients are not overheard |
| Consultation room | Assessment and treatment planning | Good, even lighting; mirror; acoustic privacy |
| Treatment rooms | Injectables, facials, skin treatments | Hand-wash basin, washable finishes, space around the bed |
| Laser and device room | Lasers, IPL, radiofrequency and similar machines | Covered windows, matt surfaces, dedicated power, extraction |
| Procedure room | Minor procedures, where offered | Treated as clinical space; clean and dirty separation |
| Recovery or rest area | Clients resting after treatment | Quiet, near treatment rooms, away from reception |
| Clean utility and sterilisation | Instruments and clean stock | Kept apart from dirty utility |
| Dirty utility and clinical waste | Used items and waste awaiting collection | Separate route, lockable, easy to wash |
| Medicine and consumables store | Injectables, fridge items, stock | Lockable, with a dedicated socket for the medical fridge |
Room sizes and adjacencies for the clinical part are partly set by KKM, so the layout is a compliance exercise as much as a design one. Get the clinical flow approved on paper before the interior is styled.
Aesthetic clients value discretion. Many do not want to be seen by the next client, so the best layouts give a short, private route from reception to the treatment corridor and, where the unit allows, a quieter exit. The consultation room needs real acoustic privacy: full-height partitions, solid doors with seals and no shared ceiling void that carries voices between rooms.
Keep staff routes, stock deliveries and clinical waste away from the client route. A small staff area with lockers and a pantry keeps personal items out of treatment rooms.
Laser and energy-based devices are the most demanding rooms in the clinic. Each machine comes with the manufacturer's installation requirements, and those, not a generic plan, set the power supply, the space around the machine and the ventilation. Ask the supplier for the site requirements sheet of every device before the electrical layout is drawn.
ClickBina's clinic guide describes clinic finishes as seamless, washable and non-porous, with coved skirtings and no-touch fittings, and names vinyl sheet flooring with welded seams and coved edges as a common choice. The challenge in a medispa is to achieve that without the clinic looking like a hospital.
Services take the largest share of a clinic budget; the clinic guide puts M&E and equipment services at the biggest share. For a medispa, plan a hand-wash basin in every treatment room, hot water to facial rooms, dedicated sockets for devices and the medical fridge, data points at every workstation and treatment room, and an aircon design that copes with machine heat and closed doors.
Medicines and fridge items must be stored securely and at the right temperature. Put the fridge on its own socket, not a shared power strip, and think about backup power for it. Check with the unit's landlord what electrical supply is available before signing, because heavy device use can exceed a small retail supply.
According to ClickBina's clinic guide, a clinic needs KKM approval or registration, local-council building and premise approval, and Bomba fire-safety sign-off, and KKM is the gating requirement and the most detailed. Mall units add the landlord's fit-out manual, hoarding and working-hour rules. Our commercial renovation permit guide explains the council and Bomba side. Confirm the current KKM requirements for the procedures you plan with KKM or your consultant before the design is fixed.
